[Fix] Persistently failing stream is retried forever - #2111
When the provider ends a stream mid-response (e.g. OpenRouter + Gemini: "Provider ended the request. Upstream idle timeout exceeded."), recursivelyMakeClineRequests pushed the same request back onto its stack with no upper bound. Without auto-approval the retry was immediate and silent (no backoff, no prompt); with auto-approval it backed off up to 10 minutes between attempts, indefinitely. A persistent upstream timeout therefore looped for hours.
Fix
Mid-stream failures now retry automatically at most MAX_MID_STREAM_AUTO_RETRIES (5) times, with the existing backoff, and only under auto-approval. Without auto-approval, or once the cap is reached, the user is asked via api_req_failed (same flow as the existing OutputTokenLimitError handling); confirming restarts with a fresh attempt count, declining stops the task.
Not covered
This does not fix the upstream timeout itself (provider pinning / reasoning effort remain the mitigations); it ensures the task stops looping and surfaces the error. Repetitive-reasoning detection is separate (#1904).
Tests
New it.each covering the auto-approval cap and the no-auto-approval prompt.
Existing tests that relied on the silent mid-stream retry now confirm the retry via a mocked ask.
core/task suites (922 tests) and check-types pass.
Mid-stream failures now trigger at most five automatic retries before the task asks for confirmation.
If automatic retries are disabled, the task asks before retrying. Declining ends the task with an error; approving retries and resets the automatic retry count.
Cancelling during retry delays stops the task, and approved retries no longer duplicate the user’s message in history.
Task limits automatic retries for mid-stream failures to five. When automatic approval is disabled or the limit is reached, Task asks the user before retrying. Tests cover retry counts, backoff, approval, and API history.
Changes: Mid-stream retry handling
Layer / File(s)
Summary
Retry limit and approval flow src/core/task/Task.ts
Task tracks automatic retries separately from history retry attempts. It applies exponential backoff for automatic retries and prompts the user when automatic approval is disabled or five automatic retries have occurred. Approval resets the automatic retry count; declining throws API request failed.
Tests verify six total attempts and five backoffs for automatic retries, one attempt before a prompt when automatic approval is disabled, and that a confirmed retry does not duplicate the user message in API history. Existing mid-stream retry tests now confirm user approval.

The PR caps automatic mid-stream retries and prompts the user, which fixes the reported endless retry loop. One narrow follow-up remains. After five failed automatic retries and an approval, a context-window error on the first chunk can retry without truncating or stopping. A test for approving at the cap is also missing. Neither blocks the main fix, but both are worth addressing.

Explanation
The description links issue #2107, explains the problem and implementation, states the scope, and summarizes the tests. It does not include the full checklist or explicit repeatable test procedure, but the required core information is present.
Full details: Out of Scope Changes check
Explanation
The production change in src/core/task/Task.ts addresses the linked stream-failure loop by limiting automatic retries, applying existing backoff, prompting through api_req_failed, and resetting the automatic counter after confirmation. The tests in src/core/task/__tests__/Task.spec.ts cover the cap, the prompt, and history behavior. No unrelated changes are present.
Full details: Security Boundaries
Explanation
No changed path violates the security boundary. In src/core/task/Task.ts, mid-stream retries use the existing autoApprovalEnabled state, stop after five automatic retries, and call ask("api_req_failed", ...) when approval is required. The retry path only reissues the API request; it does not execute commands, tools, or allowlisted file actions, and it introduces no secret or PII handling. The test changes cover both auto-approval and user-confirmed retry paths.
Full details: Persistence Integrity
Explanation
No changed persistence path meets the failure condition. The PR changes retry bookkeeping and prompt flow. The initial user-history write remains awaited at Task.ts:4008-4010, and partial stream state remains saved with an awaited saveClineMessages() at Task.ts:4098-4101. Confirmed retries set retryAttempt above zero at Task.ts:4698-4703, so shouldAddUserMessageToHistory skips a duplicate user message. No new unawaited, non-atomic, or missing partial-failure persistence operation is introduced.
Full details: Lifecycle Resource Cleanup
Explanation
No changed lifecycle leak or post-cancellation duplicate work is evident. The new retry path in Task.ts:4679-4731 caps automatic retries, awaits the existing backoff, checks this.abort before pushing another stack item, and routes cancellation to abortTask(). The stream finally at Task.ts:4737-4741 clears streaming state and the request abort controller. User confirmation uses the existing ask() cleanup path, which clears ask timers in its finally block. Existing cancellation coverage also exercises retry-stream cleanup and cancellation during backoff.
Full details: Linked Issues check
Explanation
Issue #2107 expects the affected Gemini/OpenRouter task to complete thinking and coding without the provider error. src/core/task/Task.ts only bounds mid-stream retries, then sends api_req_failed; a declined prompt stops the task, and a confirmed prompt starts another attempt. The tests verify this bounded failure flow in Task.spec.ts. The change does not prevent or recover the upstream idle timeout, so it does not meet the issue's expected result.
Resolution
Implement a provider or request-flow change that prevents or recovers the affected Gemini/OpenRouter idle-timeout failure and permits the task to complete. Keep the retry bound as a fallback if required.
Full details: Regression Evidence
Explanation
The cap-decline path is covered, but the cap-confirm path is not. Task.ts:4685-4703 asks at midStreamAutoRetries >= 5, then must retry with midStreamAutoRetries: 0 while preserving a positive retryAttempt. The new confirmation test at Task.spec.ts:672-706 confirms only the first manual failure, then declines on the second; it never reaches the cap. The auto-cap test always returns noButtonClicked, and the output-token confirmation test covers a separate branch.
Resolution
Add a focused mid-stream test that fails six times with auto-approval enabled, returns yesButtonClicked at the cap, and verifies the resumed request. Make the resumed stream fail once more and assert that it receives an automatic backoff at attempt 0, proving the mid-stream counter reset, while also verifying the history retry attempt remains positive and the user message is not duplicated.

Assert that the declined retry rejects.
recursivelyMakeClineRequests throws new Error("API request failed") when the user declines, including after the automatic retry cap is reached. The current .catch(() => {}) hides this rejection. Assert it explicitly:

Keep the history attempt separate from the automatic retry count.
On a confirmed retry, this stack item reuses currentUserContent but sets retryAttempt to zero. Line 4014 then passes the same retry state that added the first user message to shouldAddUserMessageToHistory. A non-empty request can append that user message again. Keep the history attempt positive while resetting a separate automatic-retry count. As per path instructions, check “persistence and lifecycle invariants” and “safe restart/resume without lost or duplicated state.”

A mid-stream failure (e.g. OpenRouter 'Upstream idle timeout exceeded')
re-queued the same request with no limit. Without auto-approval it did so
immediately and silently, and with auto-approval it backed off indefinitely.
Retry automatically at most 5 times, then ask the user via api_req_failed.

After five automatic mid-stream retries, the approval path pushes retryAttempt as 6. attemptApiRequest then skips truncation because 6 < MAX_CONTEXT_WINDOW_RETRIES is false. With auto-approval enabled, the fallback recursively retries without a separate limit.
Keep the positive history attempt, but use an independent counter for context-window recovery. Increment that counter only for context-window retries and reset it for the new approved attempt.
The automatic mid-stream path already incremented retryAttempt before this PR. The PR adds the approved path that preserves this inflated value, so the shared-counter issue is pre-existing but remains actionable in the new capped-retry flow.

Review comment at @src/core/task/Task.ts at line 4701:
Update the approval retry flow around retryAttempt so history attempts remain
positive without controlling context-window recovery: track context-window
retries with an independent counter, increment it only for those retries, and
reset it when starting a newly approved attempt. Ensure attemptApiRequest
applies its recovery limit to that dedicated counter.

Fixes #2107
Problem
When the provider ends a stream mid-response (e.g. OpenRouter + Gemini: "Provider ended the request. Upstream idle timeout exceeded."),
recursivelyMakeClineRequestspushed the same request back onto its stack with no upper bound. Without auto-approval the retry was immediate and silent (no backoff, no prompt); with auto-approval it backed off up to 10 minutes between attempts, indefinitely. A persistent upstream timeout therefore looped for hours.Fix
Mid-stream failures now retry automatically at most
MAX_MID_STREAM_AUTO_RETRIES(5) times, with the existing backoff, and only under auto-approval. Without auto-approval, or once the cap is reached, the user is asked viaapi_req_failed(same flow as the existingOutputTokenLimitErrorhandling); confirming restarts with a fresh attempt count, declining stops the task. Not covered
This does not fix the upstream timeout itself (provider pinning / reasoning effort remain the mitigations); it ensures the task stops looping and surfaces the error. Repetitive-reasoning detection is separate (#1904).
